Annual Health Checks

You have a right to an Annual Health Check and to any reasonable adjustments to the practice where you attend so that you can attend with confidence. As a citizen with a learning disability you can be seen by your Doctor every year. Your Doctor will do a full physical check to make sure you are OK and they may send you for further tests if you need them. You will be told If everything is good or if you need to take some actions to help improve your health. They will also ask you and or your carer about your Mental Health & Wellbeing which includes information about your thoughts, feelings and behaviour. If you require further tests or any help or advice or support this will also be written in your care plan so that you and or your carer understand what actions are required.
